Labplus S.A. conducts healthcare activities in the following type: Internal Diseases Clinic – telemedicine.

Within the scope of its operations, Labplus S.A. provides healthcare services in the area of HC.1.3.5 Other ambulatory care provided at the patient’s home in the field of 07 Internal Diseases.

The initial provision of a patient’s medical records is free of charge. For any subsequent provision, a fee is charged upon application for access to the medical records.

Rates for providing access to medical records:

one page of an extract or transcript of medical records – 13.00 PLN
one page of a copy of medical records – 0.40 PLN
medical records on an electronic data carrier – 2.50 PLN

In the event of sending medical records by traditional mail, the fee is:

domestic registered mail with confirmation of receipt – 12.30 PLN
international registered mail within the EU with confirmation of receipt – 26.00 PLN

Healthcare services provided by Labplus S.A. are not reimbursed by the National Health Fund (Narodowy Fundusz Zdrowia)1.

Healthcare services provided by Labplus S.A. are payable or free of charge for the patient in accordance with the service agreement concluded by Labplus S.A.

In the case of a patient’s payment for a healthcare service, the price for the medical service is presented to the patient for acceptance before the conclusion of the agreement with the obligation to pay for the performance of the service in electronic form.

Labplus S.A. is an entity entered into the register of entities conducting healthcare activities, providing medical services in the form of telemedicine via telecommunication and information (ICT) systems to patients, entities performing medical activities, and entities performing activities in the field of laboratory diagnostics.

The medical service is provided by Labplus S.A. using LabTest Checker by Labplus® medical software and includes the provision of medical services consisting of: interpretation of the patient’s laboratory test results and the transmission of recommendations for further laboratory tests to the patient, aimed at broadening diagnostics, and thus, in particular, the prevention, maintenance, life-saving, restoration, and improvement of health.

The operation of LabTest Checker by Labplus® is supervised by the medical team of Labplus S.A.
